Benefits

The Multi Union Security Trust Fund offers a comprehensive range of benefits. These benefits are available to the members of Teamsters Locals 166 and 986 through collective bargaining with their employers.

To enroll in the benefit plan you must complete the enrollment application that is provided to you by your employer at the time you become eligible for such benefits.

The Trusts provides benefits in accordance with the terms of the policies and agreements issued by the following providers: Anthem Blue Cross (Prepaid Medical,Prescription Drug, Mental Health, Employee Assistance Program, Life, Dependent Life and Accidental Death and Dismemberment Plan), Dental Health Services (Dental Plan), and Davis Vision (Vision Plan).

Additionally, the Trust gives you increased flexibility through the OPTIONS Rider. This plan is specifically designed to complement your HMO coverage. It provides you and your covered family members the opportunity to receive selected outpatient services from any physician. It also covers some services that are not normally covered under your HMO benefit plan. The OPTIONS Rider is not meant to replace your HMO coverage and those services provided through your Primary Care Physician and selected hospital.

Medical Benefits


Anthem Blue Cross provides the HMO medical benefit through the Multi Union Security Trust. When enrolling you must select and list your Primary Care Physician on your enrollment form. Each family member may choose a different Primary Care Physician. To find an Anthem Blue Cross provider go to www.anthem.com/ca then click Find a Doctor. Employee Assistance Progaram (EAP)


Anthem Blue Cross provides the Employee Assistance Program through the
Multi Union Security Trust and includes many services including Counseling, Legal & Financial Assistance, ID Recovery & Protection, Discount Savings Center just to name
a few of its wide variety of support and resources related to daily living.

The EAP also coordinates the Department of Transportation (DOT) and
Non-DOT Substance Abuse programs. You may access these programs by calling
800-999-7222 and identify yourself as a participant in the Multi Union Security Trust.

Dental Benefits


The Multi Union Security Trust Fund offers dental coverage through Dental Health Services (DHS). These benefits are available to you and your family as part of your benefit package. Most basic proceedures are covered at 100%. To view the complete listing of coverages select the schedule of benefits link below.

Vision Benefits


The Multi Union Security Trust Fund offers vision benefits through the Davis Vision Plan. This benefit is available to you and your family as part of your benefit package. You do not have to complete anything for your family to be enrolled in the plan.

When you are ready, just select a provider from the Davis Vision list. Simply call them for an appointment. Tell them you have Davis Vision and give them your Social Security number. Your co-payment is $5.00 for the exam. Frames, lenses or contacts may be obtained every 12 months.

Mental Health


Refer to your Anthem Blue Cross Medical Plan for your Mental Health benefits.

Options Rider


The Options Rider is a supplemental benefit for outpatient services such as physician office visits. Specifically excluded are inpatient services, major surgical procedures and maternity care in any setting. The Benefit Summary on the link below provides details about your coverage.

Life and AD&D Benefits


The Multi Union Security Trust Fund offers Life and Accidental Death and Dismemberment coverage through Anthem Blue Cross Life and Health Insurance Company. This benefit is available to you and your family as part of your benefit package. Below is a Summary of your benefit. You have 31 days from the date your life insurance through Anthem terminates to apply for individual coverage. You must complete one of the Conversion Forms and submit it to Anthem with your first month's payment. Read the Conversion Form for more information.
