Keep PacFed Up-to-Date on Critical Eligibility Info
You and your family members are entitled to some of the best health benefits in the country, but proper documentation has to be on file with PacFed Benefit Administrators before you can use those benefits to the fullest.
This is true at all times, whether it’s during your company’s Open Enrollment period or when you’re undergoing a change in life circumstances, such as a marriage, divorce, birth or adoption.
For example, if you get married, your new spouse may be eligible to share in your benefits. As soon as you say “I do,” obtain an Add/Change Form from your employer’s HR department or the Trust Fund Administration Office, and return the form with a copy of your marriage certificate within 30 days of the marriage date.
If your family is expecting a baby, adopting a child or having a child placed for adoption, send a completed Add/Change Form, along with a copy of the birth certificate or adoption papers, to the Trust Fund Administration Office to begin coverage.
You must contact PacFed within 30 days of the birth of a new baby to ensure that the baby is covered. Your new baby is automatically covered by your medical benefits for the first 30 days only.
If you have a domestic partnership, certain documents are required to add a partner to your plan. Contact PacFed to determine which forms are required for your domestic partnership.
