Make sure your beneficiary information is up to date!

In order for PacFed to best serve your needs, your current and accurate personal information must be on file. And while you may not think about it all the time, whom you designate as your beneficiary is crucial information.

Beneficiary forms are a straightforward tool for heirs to bypass the probate process and receive their funds in a timely manner. They’re also important to your Trust Fund.

But sometimes people forget what they’ve written on the enrollment forms and fail to update them when they should have.

You should fill out a new form if you’ve had a birth, death, marriage or divorce in your family. If you can’t find your beneficiary designation form, ask for a new one. If you choose to fill out a form online, be sure to print a hard copy for your files.

Make sure important information, like your address and Social Security number, your covered dependents’ Social Security numbers and all phone numbers, is up to date. Review any communications you receive from PacFed to be sure everything is correct.

You can change your beneficiary at any time, but to do so you must fill out a new enrollment form and submit it.

Even if you have notified your union and employer of changes, you still must inform PacFed. This is not done automatically.

Failure to report any change in your information or dependent status could prevent payment of claims or result in overpaid claims that could be your responsibility for re-payment.
