WHO IS ELIGIBLE?

Below is a brief explanation of the eligibility rules contained in the Summary Plan Description and Summary of Material Modification (SMM).

You must be a regular full time employee;

  1. Regular full time employees become eligible on the first day of the month employer contributions are received on your behalf;

  2. The employee must pay his or her share, as stated in your collective bargaining agreement, whether actively at work or on an approved leave of absence.

NO SURPRISE
MEDICAL BILLS

“Surprise billing” is an unexpected balance bill. This can happen when you can’t control who is involved in your care—like when you have an emergency or when you schedule a visit at an in-network facility but are unexpectedly treated by an out-of-network provider.

 
 

HOW DO
I ENROLL?

Your employer will activate your online enrollment access, when you become eligible for benefits.

All eligible dependents must be enrolled and proof of dependent status must be uploaded to the online enrollment site. (i.e. marriage certificate / birth certificate / adoption papers).

Click the button below, to access the online enrollment site. Then, follow the instructions on how to log in. Finally, use the step-by-step enrollment tutorial, to help navigate the website.

Enter your default username:
Last 4 digits of your Social Security number followed by first 5 letters of your last name in ALL CAPS.
Example: 1234SMITH

If your last name is less than 5 letters, type in your whole last name. If your last name is more than 5 letters, enter ONLY the first 5 letters.

Enter your default password:
Birth date in mmddyyyy format

MEDICAL COVERAGE

Anthem Blue Cross provides the HMO medical benefit through the MUST Trust. When enrolling you must select and list your Primary Care Physician on your enrollment form. Each family member may choose a different Primary Care Physician.

DENTAL

MUST offers dental coverage through the Dental Health Services (DHS). These benefits are available to you and your family as part of your benefit package. Most basic procedures are covered at 100%. To view the complete listing of coverages select the schedule of benefits link below.

Select a provider from the DHS list when you initially enroll. When you are ready, just simply call the provider listed on your DHS ID card for an appointment. Tell them you have DHS and give them your Social Security number.

VISION

MUST offers vision benefits through the Davis Vision Plan. This benefit is available to you and your family as part of your benefit package. You do not have to complete anything for your family to be enrolled in the plan.

When you are ready, just select a provider from the Davis Vision list. Simply call them for an appointment. Tell them you have Davis Vision and give them your Social Security number. Your co-payment is $5.00 for the exam. Frames, lenses or contacts may be obtained every 12 months.

EMPLOYEE ASSISTANCE PROGRAM

Anthem Blue Cross provides the Employee Assistance Program through the MUST Trust and includes many services including Counseling, Legal & Financial Assistance, ID Recovery & Protection, Discount Savings Center just to name a few of its wide variety of support and resources related to daily living.

The EAP also coordinates the Department of Transportation (DOT) and Non-DOT Substance Abuse programs. You may access these programs by calling 1-800-999-7222 and identify yourself as a participant in the Multi Union Security Trust.

GUEST MEMBERSHIP

As an Anthem Blue Cross HMO member, you have access to health care benefits across the country. To meet the different health care needs of members and dependents who are away from home, your Anthem HealthKeepers coverage includes benefits for covered family members who have extended stays outside the service area through the Guest Membership program. This program enables an HMO member to be a “guest” of a participating Blue Cross and Blue Shield HMO while away from their home service area for 90 consecutive days or more.

OPTIONS RIDER

The Options Rider is a supplemental benefit for outpatient services such as physician office visits as outlined in the Summary Plan Description. Specifically excluded are inpatient services, major surgical procedures and maternity care in any setting. The Benefit Summary on the link below provides details about your coverage.

This Rider is offered only in conjunction with enrollment in the Plan’s HMO benefit. Claims must be submitted to the Administrative Office, along with a completed Claim Form, within fifteen (15) months of the date of service.

MUST offers Life and Accidental Death and Dismemberment coverage through Anthem Blue Cross Life and Health Insurance Company. This benefit is available to you and your family as part of your benefit package. Below is a Summary of your benefit. Beneficiary Claim Form

You have 31 days from the date your life insurance through Anthem terminates to apply for individual coverage. You must complete one of the Conversion Forms and submit it to Anthem with your first month’s payment. Read the Conversion Form for more information.
